The East Wellington Family Health Team was established in 2007 by the Ontario Ministry of Health and Long Term Care, to provide health care for area residents at our clinics in Rockwood and Erin, Ontario, Canada.
Our vision is “to improve the health and quality of life of those living in East Wellington.”
Our mission is “to be an outstanding community–based, client–focused, interdisciplinary health service provider that delivers primary health care in a timely and integrated manner.”
While we are not currently accepting new patients in either the Rockwood or Erin Clinics, we are maintaining a wait list for both locations. If you would like to be included on this wait list, please download the Patient Wait List form, complete it, print it, and drop it off at either the Rockwood or Erin clinic.
