IMPORTANT ANNOUNCEMENT REGARDING DR. MARGO MOUNTJOY’S RETIREMENT
Please be advised that I will be closing my Family Practice at the East Wellington Family Health Team (EWFHT) as of September 1, 2012. After 24 years of caring for you, it is with great difficulty that I am making this decision. Professionally, I will be developing further my international sports medicine career with the International Olympic Committee, FINA (International Aquatics Federation) and the World Anti-doping Agency. Locally, I will be pursuing a career as the Director of Student & Resident Affairs at McMaster University Medical School and will be continuing my sports medicine role at the Health & Performance Centre at the University of Guelph.
Unfortunately, Dr. Samson is unable to accept new patients. The other Rockwood doctors have full patient rosters and are also unable to accept new patients. To find a new family doctor, you can register with Health Care Connect via telephone (1-800-445-1822) or the internet (www.ontario.ca/healthcareconnect). Alternatively, you can obtain information on doctors accepting new patients in Guelph through a city-hosted web page at www.guelph.ca/doctor, or through a telephone-based recorded message at (519) 822-1260, ext. 2507.
As of September 1, 2012, if you have not yet found a family doctor, medical care can be received in Guelph at the Speedvale Medical Clinic (519-822-0100) or at the Surrey Street After Hours Medical Clinic (519-763-6201). Emergency care can be received at the GGH Emergency Room at 155 Delhi Street, Guelph. If you require medication refills, please ensure that you have your pharmacy fax the request by mid-August 2012.
If you require a summary of your medical file, please download the Medical Release form on the right. This form can also be picked up at our Rockwood Clinic. The completed form can be e-mailed to info@ewfht.ca or dropped off at Rockwood Clinic. Please note you will be quoted for the cost of this service as it is not covered by OHIP.
I will do my best to make this transition as smooth as possible for everyone. I would like to thank you for the many years of sharing your lives with me.
