Advanced Care Planning

Part One:

Do you know who will make decisions for you if you are too ill to speak for yourself? This workshop will help you identify who your Substitute Decision Maker is, what their role is and how to have conversations with them about your values, wishes and beliefs.

Part Two:

This workshop will help you identify documents that you need to have in place "just in case". If you are too ill to care for yourself or when you die, will your family know where your documents are? We will follow our Guide to End of Life Planning and Support booklet and each person will receive a copy.
